Chronic diseases account for the most common, costly, and preventable health disparities in the United States.  Illness, suffering, and early death from chronic disease typically occurs due to lack of physical activity, poor nutrition, tobacco use, and excessive alcohol consumption.  According to the Centers for Disease Control, 1 in 2 adult Americans had at least one chronic disease in 2005 and accounted for 7 out of 10 deaths.
